Theory of Change

A Theory of Change (TOC) describes the steps that link activity to impact.

The Clinical Connect TOC shows how trusted, evidence-based small-group peer education strengthens clinicians’ knowledge, capability and confidence; breaks down silos and builds a connected, resilient workforce; and delivers clearer, more consistent care for patients and whānau.
