The Clinical Connect Learning Model

Clinical Connect is a collaborative, peer-supported learning experience where clinicians come together to reflect, discuss real cases, and strengthen their practice.

Small multidisciplinary groups of GPs, Nurse Practitioners, Nurses and Community Pharmacists enable open, evidence-based, discussion led learning that supports real world decision making.

What Makes Clinical Connect Unique

Clinical Connect is designed to support meaningful, practical, and culturally aware learning. It is:

  • Independent and peer-led

  • Evidence-based and informed by local data

  • Delivered through a health equity lens

  • Multidisciplinary and relational

  • Interactive, discussion-driven, and grounded in real practice

How the Learning Cycle Works

Clinical Connect operates as a continuous cycle that supports reflection, growth and application in practice:

  1. Prepare — Clinicians receive pre-reading to build background knowledge.

  2. Participate — Small groups meet to discuss real case scenarios and share perspectives.

  3. Practice — Clinicians reflect on insights and consider how these apply in their own work.

  4. Apply — Learning is put into day-to-day clinical practice.

  5. Repeat — Ongoing topics support continuous improvement.

What a Session Feels Like:

Before the session: Pre-Reading.

During the session: Groups discuss, reflect, and explore multiple viewpoints using the learning model.

After the session: Clinicians leave with practical takeaways, increased clarity, and renewed confidence.

For clinicians:

  • Greater confidence in everyday decision making

  • Stronger networks and trusted peer relationships

  • Improved reflective practice and cultural awareness

  • A safe, supportive space to navigate complex cases

  • Practical takeaways clinicians can apply immediately

For communities:

  • More consistent, equitable care

  • Clinicians who feel supported, connected, and confident

  • Better conversations, shared decision-making, and follow-up

  • Care that reflects local realities and patient priorities

Evidence from recent impact findings shows

  • 93% of clinicians improved their clinical decision-making

  • 98% said Clinical Connect provided a safe space to learn

  • Clinicians report feeling “more connected,” “less isolated”, and “better equipped” to support their patients
